Integrated Care

Navian Hawaii’s Integrated Care Program provides specialized medical and emotional support for patients living with a serious illness.

Integrated Care alleviates the pain and challenges patients face when living with a serious but treatable condition and can end the vicious cycle of unmet care needs leading to emergency room visits, hospitalization, and rehabilitation.

Our Integrated Care Program team includes a Nurse Practitioner and Social Worker, to support patients with specialized physical and emotional care in the comfort of their own homes.

Integrated Care is not hospice care. Patients can also seek curative treatment while in our Integrated Care Program.
