Our Hospice Care Team
Navian Hospice Hawaii’s unique, interdisciplinary approach to care provides for the physical, social, emotional, and spiritual well-being of all those we serve. Because each person is different, we assemble a care team based on the patient’s individual needs and choices. Specialists may include:
Hospice Physician or Nurse Practitioner
Coordinates the patient’s care with their own physician and oversees treatment by the hospice team.

Our Hospice Care Team

Hospice Physician or Nurse Practitioner
Coordinates the patient’s care with their own physician and oversees treatment by the hospice team.
Nurse Case Manager
Manages the patient’s physical care focusing on comfort and relief from pain and other symptoms.
Social Worker
Offers family-based emotional support, counseling and assistance with benefit applications, living wills and advance health care directives.
Hospice Aide
Provides assistance with bathing, dressing and personal grooming.
Chaplain
Offers patient and family counseling.
Bereavement Counselor
Supports the patient and family to cope with grief and loss.
After-Hours Nurse
Provides night and weekend support as part of hospice care.
Volunteer
Offers personal care, respite care for family members, light housekeeping, meal prep, errand service, emotional support and companionship.
Complementary Therapies
Offers patients a variety of therapies including art and music enrichment, massage therapy, pet therapy, aromatherapy, and Healing Touch.
